There's a video from a few years ago that's gaining momentum on YouTube. It's just a local news team in San Diego screwing up badly three times in under a minute.
First they screw up the intro and put the weather girl on camera when she isn't paying attention. Then the two anchors miss their cues, and so does the field reporter after they introduce her. And after that, the camera guy films the wrong thing.
